Babysitter information sheet template for contacts, bedtimes & allergies
This babysitter information sheet template keeps your phone numbers, an emergency doctor's details and your children's bedtimes in one place for the person watching them. Fill it in before the sitter arrives.
Each contact has room for more than one phone number, so a sitter who can't reach you on the first number has another one to dial. Allergies sit on the same sheet as the bedtimes.

Can I type my details in on a computer? No, the files are print-only PDFs, so you'll write the phone numbers and bedtimes in by hand.
